An advisory panel to the U.S. Food and Drug Administration has concluded that the benefits of Grail’s Galleri blood test outweigh its risks for adults aged 50 and older.
The 7-2 vote, with one abstention, is an important regulatory step but not an approval. The FDA is not bound by outside advisers and will make the decision on whether the multi-cancer detection test may be marketed nationwide.
Panel members were unanimous that the test was sufficiently safe. They were more divided on effectiveness, voting 6-4 that the available evidence demonstrated benefit. That split captures the central regulatory problem: Galleri can look for molecular signals associated with many cancers from one blood draw, but screening a population also raises questions about missed cancers, false reassurance and the investigations triggered by abnormal results.
In one U.S. study, the test detected 35% of cancers identified within 12 months and reported specificity of 99.85%. Advisers stressed that it should supplement, not replace, established screening such as mammography and colonoscopy.
Galleri is already available under a laboratory-testing framework, but it does not have FDA approval. A full approval would put the agency behind a defined indication, labeling and evidence package and could substantially widen access.
The agency’s final decision is expected in the coming months. Until then, the panel’s vote should be read as influential advice rather than a legal authorisation to market Galleri as an FDA-approved screening test.
